About the Role:

The MDS Coordinator plays a crucial role in ensuring the accurate and timely completion of the Minimum Data Set (MDS) assessments for all residents. This position requires a strong understanding of state and federal regulations, excellent assessment skills, and the ability to collaborate effectively with an interdisciplinary team.

Responsibilities:

Oversee and manage the completion of MDS assessments for all residents, ensuring accuracy and timeliness.

Collaborate with an interdisciplinary team (physicians, nurses, therapists, etc.) to develop and implement individualized care plans based on assessment data.

Ensure accurate and timely documentation in resident medical records to comply with Medicare/Medicaid regulations.

Play a key role in reimbursement management, ensuring the facility is properly compensated based on resident needs and services provided.

Maintain a thorough knowledge of current state and federal regulations related to MDS and long-term care.

Participate in care plan meetings and provide guidance to staff on MDS completion and documentation.

Qualifications:

Current unencumbered RN license in the Commonwealth of Pennsylvania (Required)

Minimum of two (2) years of experience in related positions (Required)

Prior MDS experience (Highly Preferred)

Experience with MDS 3.0 and knowledge of Medicare/Medicaid regulations (Required)

Strong assessment skills and the ability to interpret clinical data.

Excellent communication and interpersonal skills to effectively collaborate with the healthcare team and families.

Proficient computer skills and experience with electronic medical records.

CPR certification
