Client is located in Columbia TN and the schedule is M-F 8 am to 4 pm.
Job Responsibilities:
Diagnoses: Post stroke, Diabetes, Arthritis in ankles, Left eye vision loss, Neuropathy in feet
Goals & Objectives: Assist with all transfers using a gait belt. Encourage her to sit in the living room chair as often as possible. Allow as much independence as is safe. Assist on MWF with getting a shower-help into and out and with drying off and getting dressed. Assist with her dentures if needed, Prepare and serve healthy options for meals and snacks considering her diabetes. Keep hydrated, Light housekeeping (kitchen and garbage, wipe down bathroom, laundry, bed linens), bring mail in, engage in conversation throughout shifts. Report changes as observed. May drive to Dr appts. or run errands.
ADL's: Mobility- Client can get herself up from a lying position to sitting by herself. She does have some weight-bearing ability and can stand to pivot into a transfer chair or onto her walker. She can take several steps but needs CG to walk beside her holding onto the back of her gait belt. She needs help getting into and out of a car. Her left side is weaker, so it helps her to stand on her left side for support. She will be getting OT and PT weekly to assist with this too.
Toileting: Client has no incontinence issues. She does NOT wear depends and can tell CG when she needs to go. She needs assistance to the bathroom but also needs her privacy. She may need help wiping but CG should ask first. Bathing/Dressing/Grooming: Client likes to get a shower at least every other day (MWF) whenever CG has time for it. She needs help into the shower and out as well as with drying off and getting dressed. She will likely need help washing her hair once per week. She can pick out her own clothes. She can brush her own teeth. lCaregiver need to dry and fix her hair as she wishes. Eating: Needs meals warmed up or prepared and served to her. She can feed herself and says she has a good appetite. She should be reminded to stay hydrated. She needs to be reminded to eat healthy considering her diabetes and current sedentary lifestyle Communication: Speaks very clearly. She has no dementia and is a good communicator. She can make her own needs known and CG may want to discuss with her what she enjoys (watching crime TV shows)
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
