Client location and schedule hours:
Client located in Franklin area; Schedule is M-W-F-Sat and Sun 11:30 am to 7:30 pm.

Job Responsibilities:

Social history:


Client has 2 dogs. Client is about 5'3" & weighs 110lbs. Client feeds herself 2x/day. She is a gifted seamstress & quilter. She likes to be pampered.

Diagnoses: Heart murmur, Post Right Knee replacement, 2 Past back surgeries leaving her with 10 screws & 6 metal plates in her back. No dementia, No hearing loss. History/Habit of Dehydration (won't drink water), Fall risk


ADL's:


Mobility: Moves independently throughout her home using a walker, but tires easily needing to rest periodically. She can get into a car or her shower with minimal assistance. She doesn't like to exercise, so will need to be encouraged to do 2 sets of exercises per shift & walk a little every hour.
Eating/Diet: Client eats & drinks independently and can get prepared food out of fridge, but has unhealthy eating habits. Needs help with healthy meal planning/prep & serving. She needs to be encouraged to drink water all thru shift.


Bathing/Grooming/Dressing: Client reports that she chooses own clothes & dresses independently but might could use help getting on pants while knee is still swollen. She says she showers independently with perhaps standby assist. Takes 1-2 showers per week. Has shower chair. Goes to salon for hair, files own nails.


Medication: Client only take 2 meds regularly in the morning. Pain meds are only as needed.

Note: Cliet prefers CG to drive their own vehicle if transport or errands needed.

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
