The schedule is Monday to Sunday 9 am to 1 pm

Job Responsibilities:

Social History: Client has no pets. Highly intelligent and a good communicator. had fall several weeks ago, chipping her shoulder. Clint is 5ft 5 & weighs approximately 135 lbs. she has no hearing loss. Client likes privacy & CG should not "hover" No smoking caregiver.

Diagnoses: Rt shoulder injury, High BP, Early Dementia, Prior Brain Bleed, Afibrillation, Fall risk

Goals & Objectives: Assist client to stay clean & dry thru day. (Depends) Assist with & encourage daily shower. Assist with dressing as needed. Ask, prepare meals as requested. Light housekeeping, bring in garbage can on Fri AM, Water plants, may run errands. Walk with client on any transfer & remind to use cane. Med reminders, Offer hydration, Bring in mail daily.

ADL's:


Mobility: Client walks independently with her 4-prong cane in her left hand. CG needs to walk beside on right side all transfers. Can get up independently from a seated or lying position but will need help into & out of a vehicle. Toileting: Client wears Depends at all times but needs to be reminded to change them when needed. May need help with wiping & flushing. Likes her privacy once seated. Bathing/Grooming: Client needs help undressing and into the shower onto the seat. Needs help washing and drying hair. Eating: Client needs to be asked what she'd like for meals. Communication: Client speaks clearly and can make own needs known. Has excellent receptive as well as expressive communication skills. Has a good sense of humor even though client is depressed over the situation of not being as independent.

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
