Commuter benefits. And more. Responsibilities Performs visits for an assigned caseload of patients and families in their homes, telephonically or via video. Reports changes in patient condition/symptoms, and coordinates care with oversight from the RN/advanced practice RN. Engages patients and caregivers in discussions about their goals of care, advance directives and collaboratively participates in the development of treatment plans based on the patient's goals. Participates in assisting with and giving direct patient care in conjunction with the rest of the clinical care team to ensure interventions are aligned with patients' goals. Participates in revising the nursing plan of care where appropriate. Advocates for the patient and caregivers in liaison with other care providers to ensure care is both coordinated and consistent with the patient's goals. Educates patient/family on the trajectory of illness, and use of medications. Consults with RN/advance practice RN and seeks guidance as necessary. Complies with policies, procedures and regulatory mandates. Performs other duties as assigned. Qualifications Graduate of an accredited licensed practical nursing program or accredited school of vocational nursing. Current, unencumbered license to practice as a licensed practical nurse specific to the state you are assigned to work. Valid driver's license, auto insurance and reliable transportation. Current CPR certification. Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health, palliative or hospice setting. Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.
