Job description

The Metro Public Health Department (MPHD) seeking a bilingual Prevention Services Case Managerto spearhead the implementation of overdose prevention strategies specifically within the Spanish-speaking community. This role will perform professional, technical, and analytical work promoting social services, health services, housing, public health initiatives, and other support programs. The primary objective is to establish rapport, build trust, and facilitate positive change within the Spanish speaking community. Develop a comprehensive network of resources that addresses the needs of the service population referred by the prevention education coordinators. Coordinate alternative services to identify different approaches to finding and connecting participants to needed resources. Attends conferences, seminars, meetings, etc., to promote, collaborate and gain support for the program(s). Evaluates program effectiveness. In conjunction with the research data analyst, collects data to identify aspects of successful program operations. Prepares analytical and interpretative reports for management. Develop pre and post-test questionnaires and other instruments to measure effectiveness of case management services. Conduct follow-up studies on program participation and provide a written report to the program manager on a bi-weekly basis. Participate in needs analysis studies to identify specific needs services. Assists senior staff with special assignments as needed. Keeps abreast of current developments and trends in community engagement, prevention, and intervention. Coordinate community resources with prevention education coordinators Explore and/or place clients in specialized programs as needed. (i.e. transitional services, transportation services.) Other administrative functions as directed. Writes narrative and statistical reports and letters for dissemination to all stakeholders. Maintains pertinent records and confidential information Any combination of education and experience that would prepare the incumbent to perform the duties of the position at the appropriate level. Employees would typically have a bachelor's degree and/or 5 year's professional experience.
