HealthMark Rehabilitation is an _inhouse_ therapy company, serving the Skilled Nursing Facility patients and residents of HMG facilities.

We're currently seeking a Full-time & PRN Physical Therapist to evaluate and treat residents and short-term patients at our Accel at College Station facility in College Station, Texas.

$5,000 SIGN ON BONUS (FULL-TIME)
Prefer DPT candidates but non-DPT will be considered
PRN not eligible for Sign on bonus

Responsibilities include:

Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.

Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.

Assist as needed with patient screenings for the need for Physical Therapy.

Communicate progress and any barriers toward PT goals with interdisciplinary team members.

Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.

Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

Provide and document patient, family, and caregiver training regarding each PT patient's specific plan of care and recommendations for follow up.

Perform additional duties as required.

Qualifications include :

PT degree from an accredited Physical Therapy program.

Current Texas license in good standing as a Physical Therapist.

Meet physical requirements to deliver PT responsibilities.

Experience in a geriatric rehabilitation setting preferred.

Successful completion of hiring process / background check.

This Sign On Bonus is to be paid in 4 installments. The first payment of $1,000 will be made with the next payroll after the full-time employee as documented above has successfully completed 90 days of work without any disciplinary action. The second payment of $1,000 will be made with the payroll following the completion of 6 months of satisfactory work performance at full-time status. The third payment of $1,000 will be paid upon the successful completion of 9 months of full-time employment without any disciplinary action. The final payment of $2,000 will be paid upon the successful completion of one year of employment as a full-time employee, without any disciplinary action.
