Job description

The Physical Therapist is a qualified professional who assists the physician in evaluating level of function, helps develop the plan of treatment (revising as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel, and participates in in-service programs.
QUALIFICATIONS

The Agency shall employ only Physical Therapists by the state of Agency practice.

Has graduated from a Physical Therapy curriculum approved by:
The American Physical Therapy Association.

The council on Medical Education and Hospitals of the American Medical Association.

The council on Medical Education of the American Medical Association and the APTA.

Prior to January 1, 1966
Were admitted to membership by the American Physical Therapy Association.

Were admitted to registration by the American Registry of Physical Therapists.

Has graduated from a Physical Therapy curriculum in a four-year college or university approved by a State Department of Education.

Has two years of appropriate experience as a Physical Therapist and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service except that such determination of proficiency do not apply with respect to persons initially licensed by a State or seeking qualification as a Physical Therapist after December 31, 1977.

Were licensed or registered prior to January 1, 1966, and prior to January 1, 1970, had fifteen (15) years of full-time experience in the treatment of illness or injury through the practice of Physical Therapy in which services were rendered under the order and direction of attending and referring physicians.

If trained outside the United States:
Were graduated since 1928 from a Physical Therapy curriculum approved in a country in which the curriculum was located and in which there is a member organization of the World Confederation for Physical Therapy.

DUTIES
The Physical Therapist:

Provides treatment as ordered by the attending physician to relieve pain, develop or restore function and maintain maximum performance, using physical means such as exercise, massage, heat, water, light and electricity.

Assists the physician in evaluating the level of function, disability, injury, or disease process of individual patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.

Interprets plans, establishes, and exercises proper treatment program in accordance with the physician's referral, periodically reviewing the patient's program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy treatment.

Contacts the physician directly for patient reports and care as needed.

Submits to the physician and the Agency written summary of the patient's progress at the designated intervals of every sixty (60) days and at discharge of the patient using the appropriate and designated forms.

Participates in case conference as required.

Works in collaboration with the physicians and other members of the health professional team in determining the needs of the patients and in developing their total plans of care.

Observes, records, and reports to the physician the patient's reaction to the therapy program or any changes in the patient's condition.

Instructs, teaches and supervises patients, their families and other health team personnel regarding physical therapy procedures as related to the patient's therapy programs.

Evaluates the home environment and makes appropriate recommendations.

Documents each visit made to the patient and incorporates notes into the clinical record at least weekly.

Participates in staff development activities, which may include providing in-service training to other home health personnel; participates in in-service education programs.

Supervises Physical Therapy Assistant(s) and adheres to all rules, regulations, codes of ethics, guidelines, and codes in federal, state, and local laws which are concerned with the practice of physical therapy; adheres to the policies established by the Agency.

Treat and reassess patients seen by PTA at least every 30 days.

Conduct face-to-face patient care conference with each PTA to review progress and modification of treatment program for all patients seen by PTA.

Communicate and coordinate activity progress with other members of the team.

Perform initial assessment for patients requiring only physical therapy services.

Participates in Performance Improvement Activities.

May supervise home health aides.

Performs other ordered job-related duties as assigned.

Pay: $85.00 - $125.00 per hour
