Position Overview: Under the direct supervision of the Outreach Team Lead, the Community Health Worker is responsible for supporting Harris County Public Health's outreach activities and serves as the key liaison to educate, promote, and connect community members to all HCPH programs/services specifically on behalf of the Public Health Preparedness and Response Programs. This position will primarily work in the community through various outreach activities within Harris County, while also completing in-office tasks/projects to support overall outreach team objectives. Duties & Responsibilities: Responsible for diverse outreach, education and community coordination activities such as canvassing local businesses/organizations, attending community events, conducting outreach phone calls, and giving presentations. Serves as a subject-matter expert to meet outreach objectives for HCPH programs and recommend changes to improve outreach strategies and support the creation/updating of HCPH program trainings for the entire outreach team. Maintains knowledge/skills to educate, promote, and connect community members to all HCPH programs/services. Tracks, collects, and submits outreach service data on a daily and weekly basis. Builds and maintains relationships with stakeholders and organizations to create and sustain innovative partnerships. Responsible for initiating referrals to HCPH programs/services, while ensuring community members' confidentiality. Creates and delivers educational presentations/workshops on public health topics for diverse target audiences. Days and hours of work are typically Monday through Friday, from 8:00 am to 5:00 pm. Other days including weekends, and evening hours as necessary for Outreach needs and to attend community events. This position requires the ability to travel within Harris County jurisdiction. Work Environment: This job operates in both field and office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Physical Demands: This is a largely stationary role, however does require field work and many duties would require the ability to lift up to 20 pounds and bending and standing for periods at a time. Has the ability to enter and correct data, modify a workbook, format a worksheet, and use printing functions. Understands the different database concepts and structures and is familiar with data validation and is able to create a simple presentation in PowerPoint, run it, and print it. Entry Level: A person with this level of skills has the ability to open, create, save and modify documents in Word, send and receive email in Outlook and create spreadsheets in Excel. Format documents for printing, comfortable using the printer menu to preview documents. Has ability to change the font, the margins, insert or delete pages and use the built-in spellchecker and grammar check.
