"AAMA's Prevention & Counseling Minorities Action Program is seeking a self-motivated, organized, highly detailed, and goal-orientated Prevention Navigator for the funded project of the SAMHSA FY 2023 MAI funded opportunity of SP-23-005 Prevention Navigator. The overall responisbilities are to provides navigation services to link eligible individuals to support services, including STIs, PrEP/PEP, housing, mental health, substances use treatment, and other service needed by participants, and conduct follow-up to ensure individuals are encouraged and retained in care and treatment. The purpose of this program is to provide substance use and HIV prevention services to racial and ethnic minority populations at high risk for substance use disorders (SUDs) and HIV infection. The program places emphasis on men who have sex with men (MSM) and men who have sex with men and women (MSMW), as well as those who identify as Lesbian, Gay, Bisexual, Transgender, Queer/Questioning and Intersex persons (LGBTQI+) who are not in stable housing and/or reside in communities with high rates of HIV, Viral Hepatitis (including Hep A, B, and C), and/or Sexually Transmitted Infections (STIs). With this program, SAMHSA aims to build a solid foundation for delivering and sustaining quality and accessible state-of-the-science substance misuse and HIV prevention services for high-risk populations within minority communities. MINIMUM QUALIFICATIONS: High school diploma or equivalent and a minimum of one-year experience in case management/linkage/navigation services with the target population of the grant. Prefer bilingual Spanish and previous experience working with grant-required populations. Proficiency in the use of Social Media, computer, and Microsoft Office/365; Word, Excel, PowerPoint Good understanding of 12-step programs if working in a chemical dependency program and knowledge about community resources Excellent oral and written communication skills Must have a valid Texas Driver's License PHYSICAL AND MENTAL REQUIREMENTS: Ability to communicate effectively orally and in writing. Ability to maintain and protect client confidentiality. Ability to remain calm and make informed, well-thought-out decisions at all times. Ability to effectively deal with and help resolve client crisis situations. Ability to collaborate well with a team of caregivers to ensure the best quality care for clients.
