Summary:

We are seeking a caring and compassionate Licensed Practical Nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well-being of residents such as assist in admission of new residents; assist with activities of daily living when needed; administer specified medications; supervise non-licensed nursing staff and participate in the evaluation of non-licensed staff.

Duties and Responsibilities include the following. Other duties may be assigned.
1. Demonstrates excellent decision-making skills and can understand the impact of those decisions.
2. Sets and manages priorities.
3. Accepts calls from staff during nights and weekend.
4. Meets with oncoming and departing licensed practical nurse to discuss resident's condition and any occurrences from the shift.
5. Responsible for making sure communications are clear, accurate and timely.
6. Counts controlled substances with off going nurse, assumes responsibility for assuring accuracy of count before accepting keys.
7. Checks, orders, and administers prescribed medication with 100% accuracy.
8. Responsible for always ensuring the correct dosage by never pre-pouring medications.
9. Demonstrates knowledge of important medication interactions and side effects.
10. Ensures medication cart is locked at all times.
11. Obtains pertinent information and completes all required forms as established by policy and procedure and ongoing thereafter, upon resident admission.
12. Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare providers orders
13. Assists in completing evaluations when needed for residents.
14. Provides hands on care on an as needed basis to meet the needs of the residents.
15. Works with the ADORS in reviewing and investigates all incidents reports and recommends appropriate action and follow up to the DORS/executive director.
16. Understands and adheres to the ALF regulations pertaining to assisted living health care laws.
17. Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.
18. Responsible for always practicing good body mechanics and safe working habits.
19. Knows, understands, and follows established policy and procedures in emergency situations.
20. Takes time to give associates in nursing daily feedback on his/her performance.
21. Participates in the associates performance evaluations when request by the DORS.
22. Responsible for ensuring associates participate in activities provided to residents as described in the activities calendar.
23. Responsible for always responding promptly and courteously to resident's request for assistance including phone calls and request from family and visitors.
24. Ensures there is a safe, clean and orderly environment for all residents.
25. Responsible for always ensuring and supporting a dignified and caring atmosphere for residents, families, visitors and associates.
26. Responsible for always wearing clean and appropriate dress per established policy.
27. Understands and complies with HIPAA regulations.
28. Responsible for being considered essential personnel especially during emergencies.
29. Takes the initiative in work, seeks out additional information and offers solutions/suggestions.
30. Performs all other reasonable duties as assigned or as requested.
31. Responsible for willingly taking on special projects or assignments outside of regular duties.

Supervisory Responsibilities:
Directly supervises employees in the Resident Services/Nursing Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Qualifications:

Language Ability:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Math Ability:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills:
To perform this job successfully, an individual should have knowledge of word processing software, and electronic medical records.

Education/Experience:
High school diploma or general education degree (GED); and one to two years related experience and/or training; or equivalent combination of education and experience.

Certificates and Licenses:
Valid Florida nursing license
