Care Advantage Skilled a Care Advantage Inc company, (Formally known as All About Care) Voted in Home Healthcare the last 5 years is looking for a Licensed Practical Nurse (LPN) to join our skilled nursing team in a Full-Time capacity.

Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care. This position promotes and supports the Company's core values of Compassion, a positive Attitude, Respect and Excellence.

Our Licensed Practical Nurses assist with all (home based) skilled nursing care based off the plan of care for patients located within the Goochland, Mineral, Bumpass and Sandy Hook VA territory.

The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide skilled nursing care to patients on an intermittent basis.

Required Qualifications:

Graduate of an approved school of professional nursing
Current and valid Virginia LPN License
Passionate and motivated to provide care within a patient's home.
Ability to exercise initiative and independent judgment.
Comfortable working collaboratively in cross-functional teams
Ability to communicate effectively, both verbally and in writing, with all levels of an organization
Ability and willingness to travel within a given territory.
Willingness to work with multiple patients in varying home locations.

Preferred Qualifications:

Two (2) years' home health nursing experience, preferred.
Previous experience with Home Care Home Base EMR system strongly preferred.
Wound Care experience or Certification strongly preferred.
