Franciscan Villa has an exciting opportunity for an RN MDS Coordinator.

The RN MDS Coordinator plays a key role in ensuring the accuracy of resident assessments and the optimization of care plans. Key responsibilities include:

  • Leading and coordinating the development and completion of resident assessments (MDS) in compliance with current regulations and guidelines.
  • Ensuring the MDS process is completed accurately and within required timeframes.
  • Collaborating with the interdisciplinary team to create personalized care plans for residents.
  • Continuously monitoring and documenting residents' progress and responses to care plans.
  • Actively participating in the Quality Assurance and Performance Improvement (QAPI) process to enhance care quality.
  • Communicating effectively with residents, families, and healthcare providers to ensure seamless care coordination.
  • Staying informed on MDS regulations and guidelines to ensure compliance with industry standards.

This role is essential in maintaining high standards of care and regulatory compliance within the facility.

Wage: Up to $84,000 annually

Requirements:

  • Current RN (Registered Nurse) license
  • RAC-CT (Resident Assessment Coordinator-Certified) certification is required
  • Strong understanding of MDS 3.0, RAI process, and care planning
  • Excellent organizational, communication, and interpersonal skills
  • Ability to work independently and collaboratively within a team
  • Commitment to providing high-quality care to residents
