Ascendo Healthcare Staffing is seeking a dedicated and compassionate Home Health Occupational Therapist to join our team . The ideal candidate will have a strong background in providing therapeutic services to a diverse patient population, including pediatrics, geriatrics, and individuals with disabilities. This role involves assessing patients' needs, developing individualized treatment plans, and implementing therapeutic interventions to enhance their ability to perform daily activities and improve their quality of life.

Are you a compassionate and skilled Occupational Therapist looking to make a difference in patients' lives? In this role, you'll provide exceptional care by visiting multiple patients per day, helping them regain independence and improve their quality of life.

Responsibilities:
Conduct comprehensive patient assessments in a home health setting to develop and implement personalized therapy plans.
Provide therapeutic interventions to help patients improve mobility, strength, and daily living skills.
Utilize OASIS & HCHB (HomeCare HomeBase) documentation to accurately evaluate and record patient progress and outcomes.
Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care.
Educate patients and caregivers on therapeutic techniques and strategies to support recovery and independence.
Travel to multiple patient homes daily (average of 5-6 visits per day).

Qualifications:
Current and active Occupational Therapist license in the state of practice. (WI)
Minimum of 1+ year of Home Health experience within the last three years.
Proficiency with OASIS documentation and HomeCare HomeBase (HCHB) EMR system.
Strong communication and organizational skills, with the ability to manage multiple patients daily.
