Description:
The Manager, Community Health Programs is responsible for overseeing development of community health programs to support Emory Healthcare care delivery, community health improvement plans and other strategic initiatives.
The manager will develop and implement processes and procedures so that programs can be deployed in various settings with unique objectives, while ensuring high quality standards and accountability.
The ideal candidate will have strong organizational skills, a deep understanding of social determinants of health and the communities we serve, and a commitment to improving health outcomes in underserved communities.
KEY RESPONSIBILITIES:
1. Program Development and Administration
Oversee the day-to-day administration of community health workers and other community health programs, ensuring alignment with the goals of the Office of Health Equity and the Community Health Improvement plans.
Coordinate the planning, implementation, and evaluation of community improvement initiatives aimed at addressing social determinants of health.
Manage program budgets, resources, and staff to ensure effective and efficient program operations.
Develop and maintain program documentation, including policies, procedures, and reports.
2. Support for Social Determinants of Health Initiatives
Assist in the development and implementation of strategies to address social determinants of health, such as housing, education, employment, and access to healthcare.
Coordinate with internal and external stakeholders to integrate social determinants of health into community health programs and services.
Monitor and evaluate the impact of these initiatives on health outcomes and provide recommendations for program enhancements.
Identify opportunities for continuous improvement in community health programs, including the use of new technologies, approaches, and partnerships.
Stay informed about best practices and emerging trends in community health and social determinants of health and apply this knowledge to enhance program effectiveness. - Contribute to the development of innovative solutions that address barriers to health equity in the community.
3. Data Management and Reporting
Collect, analyze, and report on data related to community health programs and initiatives, including participation rates, health outcomes, and community feedback.
Prepare regular reports for internal leadership, community partners, and funders, highlighting program achievements and areas for improvement.
Ensure compliance with all data privacy and security regulations in the management of community health data.
MINIMUN QUALIFICATIONS:
Bachelors degree in Public Health, Health Administration, Community Health, Social Work, or a related field.
A Masters degree is preferred
Experience:
Minimum of 5 years of experience in community health program management, administration, or a related role within a healthcare or community-based organization.
1.Experience working on initiatives that address social determinants of health and promote health equity.
2. Supervisory experience with both paid staff and volunteers.
3. Proven experience in community engagement and partnership development.
