Job description

Position Summary: The behavioral health provider (BHP) is a member of the clinic's health care team who assists the primary care provider (PCP) in managing the overall health of the clinic population. The BHP's goals are to help improve recognition, treatment, and management of psychosocial/behavioral problems and conditions in the enrolled population. The BHP will provide consultation services to all patients referred by the primary care team. The person in this position will deliver consultation-based services in the primary care clinic to patients and PCPs using an integrated care model. Ability to apply the biopsychosocial model of assessment to the primary care setting. Due to the nature of the clinic, this position is a dual role between BHP and Mental Health Counselor. Mental Health Counselor (MHC) will counsel individuals and groups to promote optimum mental health. The MHC will help individuals deal with thoughts of suicide; addictions and substance abuse; family, parenting, and marital problems; stress management; problems with self-esteem; and issues associated with aging and mental and emotional health.

Qualifications for BHP/MHC:

• Master's degree in psychology, counseling, social work, or a related field required.• Appropriate state credentials and licensing required.

The BHP/MHP duties/responsibilities:

• Ability to formulate diagnostic and treatment recommendations and present findings to treatment teams (i.e., physicians, social workers, psychologists, and nursing staff as appropriate), and knowledge of and skill in the uses of psychological assessment, psychodiagnostics evaluations, and psychotherapy. • The scope and responsibilities include targeted assessment and evaluation, including diagnostic impressions and functional status focused on the presenting problem.• BHP shall provide timely and succinct feedback to primary care providers regarding consultation findings and recommendations.• Counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes. Maintains confidentiality of records relating to client's treatment.• Guides clients in the development of skills and strategies to deal with their problems. Encourages clients to express their feelings and to discuss what is happening in their lives and helps them develop insight into themselves and their relationships.• Collects information about clients through interviews, observations, and tests. Prepares and maintains treatment records and reports.• Evaluates clients physical or mental condition based on review of client information. Develops and implements treatment plans based on clinical experience and knowledge. Acts as a client advocate to coordinate required services or to resolve emergency problems in crisis situations.

Pay: $35.00 - $40.00 per hour
