CDD is a non-profit community-based organization that supports individuals with developmental disABILITIES in their own homes throughout the Kansas City Metropolitan area. We have been doing this for over 50 years with the goal of adding 50+ more in our sight. Speaking of goals, CDD offers incentives based on meeting agency-wide goals as well as differentials for homes that are behavioral, high medical needs, and for shifts that occur on Saturday and Sunday.

All management must have a Class E driver license in Missouri or a Class C driver license in Kansas, proof of auto insurance in your name for a reliable and legal form of transportation, and proof of education.

Essential Functions:

  • Directly supervises, trains and evaluates Residential Specialist for assigned locations;
  • Implements and monitors individualized services based on Person Centered Support Plan (PCSP) principles;
  • Acts as the direct contact person with service coordination entity for ensuring that PCSPs are in accordance with Missouri Quality Outcomes and that pertinent information is included;
  • Ensure PCSP meetings are scheduled and is prepared to chair annual planning meetings with the support coordinator;
  • Submits all Annual Plan-related documentation according to Annual Plan schedule;
  • Maintains Case Record Book (CRB) and Medical Record Book (MRB);
  • Completes discharge summaries and follow-up of individuals leaving the agency;
  • Visits each location monthly to complete a monthly Site Visit form to review resident progress, medical records and documentation in Therap, case records, resident money ledgers and cash balances, medications, health and safety related areas, etc.;
  • Completes monthly directives by the assigned due date and submits to the Residential Specialist (RS);
  • Completes, updates, and submits monthly reviews to Support Coordinators;
  • Assist the QAD with completing peer reviews as requested;
  • Uploads completed MARs and POs from the previous month into Therap;
  • Runs weekly T-Log reports to check percentage compliance and communicates to team the areas for improvement; ensure staff are held accountable to agency documentation policy;
  • Takes the lead role with completing HIPS/APTS in accordance with DMH required timelines;
  • Submits required documentation for nursing audits;
  • Group Home PS competes CIST notes as required;
  • Coaches program staff in behavior management, crisis intervention, behavioral counseling, along with monitoring the protection of resident's rights;
  • Is responsible for disciplinary matters related to the PCSP and also assists the Residential Specialists with appropriate disciplinary action related to policy and procedure violations;
  • Ensures that the Lead Direct Support Professional (DSP) submits relevant sections of monthly reports, eg. Emergency drills, meeting minutes, vehicle use, etc;
  • Provides monthly in-service trainings at each location and facilitates staff development as required to ensure quality care and services;
  • Conducts all in-home skills training to DSPs as needed;
  • Serves as the contact person for relaying house/programmatic information to the Program Director;
  • Alters personal schedule when needed to observe all program operations;
  • Attends meetings as required at each location;
  • Assist with completing and reviewing staff evaluations with Residential Specialist;
  • Is on-call for the occasional or extended absence of a Residential Specialist
You would probably be great for this role if you have the following:
  • Bachelor's degree in education, rehabilitation, social or behavioral science field with minimum 2 years of supervisory experience with persons with developmental disabilities;
  • Master's degree in education, rehabilitation, social or behavioral science field preferred with two years program management and staff supervisory experience;
  • Be at least 21 years old
  • Valid driver's license and a clean driving record
  • Ability to pass medication, adult CPR and First Aid classes
  • Your own car and car insurance
  • Experience in the developmental disabilities field
  • Ability to lift 10 pounds consistently, 20 pounds frequently and up to 50 pounds occasionally

In order to make this all the way legit, you give permission for CDD to conduct background screenings as your employment is contingent until results are verified. We want you to know the following will occur:

  • A request for a background check will be conducted which will include criminal history, abuse and neglect and sex offender registry
  • Motor vehicle report check
  • Education verification and license/certification checks (where applicable)
