Job description
JOB SUMMARY:
We are seeking a compassionate LCSW or LPC to join our team at Insite Health. The clinician will be responsible for providing psychotherapy and other mental health services to patients with mental health conditions. The successful candidate will have a strong clinical background, excellent communication skills, and a passion for helping patients improve their mental health.
Key Responsibilities:
Complete necessary administrative procedures at patient admission, establishing accurate treatment expectations, including service nature, program goals, procedures, conduct rules, treatment expectations, duration of care, patient rights, responsibilities, HIPAA, and PHI.
Reassess patient needs monthly or as required by significant changes, providing necessary contacts, testing results, and documentation for each treatment service.
Utilize relevant assessment information to guide treatment planning, formulate measurable goals and objectives in collaboration with patients and guardians, and develop mutually acceptable treatment plans.
Reassess and adjust treatment plans every 90 days or as needed, ensuring coordination with community resources and alignment with the patient's diagnosis and placement criteria.
Establish a supportive therapeutic relationship with the patient, encouraging engagement in treatment, promoting positive behavioral changes, and adapting evidence-based strategies to individual patient characteristics.
Work with patients to discourage behaviors inconsistent with treatment goals, making constructive therapeutic responses when needed.
Contribute as a multidisciplinary team member, ensuring collaboration with all InSite Providers and maintaining confidentiality.
Exchange and coordinate treatment information with agencies or professionals in compliance with confidentiality rules and professional standards, ensuring active ROI documentation in the patient's EHR.
Complete all documentation, including treatment plans, progress notes, and reports, according to policy, ensuring legibility and patient participation.
Anticipate and meet the needs of patients and programs, ensuring efficient programming through proactive coverage and participation.
Adhere to confidentiality laws related to substance use, professional ethics, and continuously develop personal and professional goals with supervision to enhance program effectiveness.
Requirements:
Master's degree in social work or a related field, with a focus on clinical social work
Current state licensure as an LCSW
Minimum of 1 year of experience providing psychotherapy and other mental health services to patients with mental health conditions
Excellent communication and interpersonal skills to effectively build relationships with patients and collaborate with healthcare professionals
Strong clinical skills, including the ability to conduct thorough assessments and develop evidence-based treatment plans
Experience using electronic health records (EHRs) and other healthcare software to manage patient records and coordinate care
Knowledge of mental health conditions and treatments, and an understanding of the challenges faced by patients with mental health conditions
Ability to maintain confidentiality and adhere to ethical and professional standards of practice
