Job description

The Pharmacist serves as a specialist in a selected area of pharmaceutical care with the ultimate goal being the safe, efficacious and cost effective use of drugs and information about drugs to achieve outcomes that improve patient quality of life.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Patient Experience

• Identifies opportunities to improve patient experience
Participates in projects/programs designed to enhance service delivery and patient satisfaction.

Patient/Staff/Environmental Safety

• Identifies opportunities to improve the Environment of Care, and patient and staff safety
Participates in projects and programs designed to create a safe environment to avoid error.

Specialty Function

• Provides service based on advanced knowledge or experience in a selected operational or clinical area of specialty practice.

• Specific job functions and requirements of specialty practice are listed in department description of specialty.

• Performs those functions of the Pharmacist Practitioner listed below as needed.

Direct Care Functions

• Interprets and screens medication orders to determine appropriateness of prescription, identifies duplication of drug interactions and consults with the prescriber for clarification.

• Enters medication orders into pharmacy/hospital information system accurately and efficiently.

• Prepares and dispenses medications as required in an accurate and timely manner.

• Medication orders are processed expeditiously and accurately; turn around times meet expectations; dispensing errors do not occur.

• Verifies medications prepared by technicians.

• Counsels patients on safe and appropriate use of drugs.

• Intervenes when necessary to improve the quality of patient care and ensure rational and cost effective drug therapy.

• Gathers patient specific information as necessary to accurately assess and monitor drug therapy
Reviews medical records, laboratory findings and medication administration records, and interviews patient as necessary.

• Conducts and documents provision of specific clinical services including, but not limited to, pharmaceutical consultations and interventions, patient teaching, medication histories, adverse drug reaction assessment or drug therapy review; documentation occurs in the medical record or other appropriate place.

• Assures continuity of pharmaceutical care by providing physicians, nurses and other health care professionals with necessary information related to a patient's therapeutic drug regimen.

• Communicates and participates in the clinical care of patients; documents care provided.

• Evaluates the use of non-formulary drugs and suggests formulary alternatives as appropriate.

• Documents all recommendations made to prescriber regarding appropriateness of drug therapy.

Organizational and Administrative Functions

• Supervises the daily activities of technicians and clerks in the responsible work area.

• Supports and upholds established hospital and departmental policies, procedures and guidelines, and state and federal regulations of pharmacy practice.

• Performs duties in a fiscally responsible manner.

• Participates in quality improvement, DUE and The Joint Commission compliance as required.

• Maintains a clean and organized work place.

• Assumes responsibility for the department in the absence of supervisory personnel.

Educational Functions

• Orients and trains new employees as requested.

• Will function as a preceptor after one year of employment.

• Provides in-service education on medication use to other health care professionals and/or pharmacy staff.

Competency Enhancement

• Keeps current on latest developments and practices of pharmacotherapy.

• Maintains competency and enhances own professional growth through continuing education, conferences, in-services and professional society participation.

• Maintains positive communication, involvement and commitment to interpersonal customer-centered service toward patients, physicians, nurses and co-workers.

• Maintains competency and knowledge in the assessment, treatment and care of patients of all ages, including neonates, pediatrics, adolescents, adults and geriatrics with respect to pharmacotherapy.

• Maintains competency and knowledge in specialty area as defined by specialty description.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:

Education

• Knowledge of pharmaceutical standards and practices normally acquired through completion of a Bachelor's or Doctorate Degree in pharmacy from an accredited school of pharmacy, with a Doctorate required for some specialty areas of clinical practice as defined by specialty description.

• Completion of a residency and/or fellowship in pharmacy practice preferred and board certification or eligibility for such in specialty area of pharmacy practice, or equivalent experience desired.

Experience

• A minimum of one years' experience desired in institutional, hospital or health system pharmacy practice to enhance and develop skills.

• Unique experience in a specialty area of pharmacy practice beneficial.

Required

• Possesses current Wisconsin licensure as a pharmacist.

• Current CPR certification or obtained within 90 days of hire (except for job code 20604) - All clinical personnel in identified job codes are required to maintain cardiopulmonary resuscitation (CPR) training through independent study, manikin and knowledge testing
An acceptable CPR training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the client Professional Rescuer Program with AED
(See Cardiopulmonary Resuscitation Training, Policy ).

Other

• Membership in the American Society of Health System Pharmacists or the American College of Clinical Pharmacy, and the Pharmacy Society of Wisconsin desired.

Annual Competencies

• Annual competencies as required by Aspirus and/or various regulatory agencies based on entity and/or job position.

Details:
Required Certs: WI Pharmacist License
Required Education: Bachelor's Degree
Minimum 3 years of inpatient hospital experience
Charting: Epic experience required
Unit Specific Skillset :able to manage multiple concurrent responsibilities in a central inpatient pharmacy verifying EPIC orders, checking technician work, answering pharmacist phone calls, etc.
Inpatient pharmacy operating from weekdays, weekends/holidays, providing inpatient and outpatient IVs, inpatient unit dose meds, and outpatient chemotherapy.
Inpatient Census usually 30-40
Inpatient and Outpatient Surgery, Med/Surg, OB/GYN, ICU, outpatient oncology, ED
Hard worker, Resilient and Manages Change well, Social candidates.
Scrub Colors: NA- Business Casual- No lab coats
